First Appointment

Visiting us for the first time? You can download your new patients form here. Just fill out the paperwork at home and then bring it with you along with your insurance card (if you have one), your driver’s license or photo ID. Or you can arrive 10-15 minutes prior to your scheduled appointment to fill out the paperwork in our office.

Insurance and Payment Options

Our office accepts cash, personal cheques, Visa, MasterCard and American Express. We also accept most dental health insurance, and we’ll file all your claims. We’ll make sure that you receive the maximum benefits available!
